Models are good and getting better. They'll keep occasionally writing perfect sentences nobody said. You can't eliminate this risk by requesting a better model.

Make the risk survivable instead. Every claim drags its source behind it. The system gets proud to say "I don't have a source." The human job becomes looking where the machine admits uncertainty.

The key question isn't "does your AI hallucinate?" All of them do. The real question is whether your system makes the lie wear a costume or raise its hand.
